COVID-19 Questionnaire
Part of our CPC process involves gathering COVID data from the community via a voluntary questionnaire that asks the patient's vaccine status, their feelings toward COVID-19 vaccines, and what they are doing to cope in COVID world.
Vaccine Info
BCCDC offers information on how to get a vaccine, who should get a vaccine, vaccine safety, proof of vaccination, and more.

We also offer COVID-19 and flu shots at our weekly CPC events.
